Hospital to home
Falls prevention assessment
A physiotherapist works out why the falls are happening, then builds a plan.
From £150
This is not an emergency service. If someone has chest pain, stroke signs, severe breathing difficulty, heavy bleeding or collapse, call 999 or go to A&E. We work alongside the NHS, not instead of it.
This is not 999
If someone is on the floor, has hit their head, cannot move a limb, or you think they have broken a hip, call 999. This service is for planned prevention, not for someone still on the carpet.
About this service
After the first fall, everything changes, confidence most of all. A physiotherapist comes to the home and works out why it happened: balance, strength, hazards, footwear, eyesight, medicines. Then they build a practical plan, exercises, equipment, changes around the house, to reduce the next one.
Who it is for
Anyone who has fallen or nearly fallen, families frightened by a first fall, and care homes wanting a clearer plan for a resident.
Is this right for me?
- There has already been a fall or a near miss
- A parent has become frightened of walking
- You want a home-hazard review, not just a leaflet
Working alongside your NHS team
We do not replace your NHS GP, district nurse, consultant or A&E. We add private capacity at home. If you already have an NHS package, tell us so we do not duplicate or confuse the plan.

A physiotherapist, with GP or nurse input if medicines or an acute injury are part of the picture.
Pricing
From-price for a physiotherapy-led falls assessment at home. Equipment and follow-up visits are quoted separately.
After your visit
Follow the exercise plan. If another fall happens, treat it as a new event, not a failure of the leaflet. We can add rehab visits or a GP review.
Where we cover
Home visits across Lurgan, Craigavon, Portadown and Banbridge as core cover, with Lisburn, Armagh, Dungannon and Belfast arranged with a travel note. Call with a postcode and we will confirm. A family member can book if the patient consents to be seen.
FAQs
Do you supply rails and frames?
We recommend what is needed. Supply is arranged separately, often through existing community equipment routes or a private provider. We will not sell you kit you do not need.
