Hospital to home
Hospital discharge support
A planned first week at home after a hospital stay.
From £275
This is not an emergency service. If someone has chest pain, stroke signs, severe breathing difficulty, heavy bleeding or collapse, call 999 or go to A&E. We work alongside the NHS, not instead of it.
About this service
The day you leave hospital is often when the real worry starts. Discharge support is a planned visit (or a short series of visits) to check medicines, wounds, mobility and what the hospital letter actually asked you to do. The hero line we use with families is simple: coming home from hospital? We are here for the first 7 days.
Is this right for me?
- Someone coming home this week
- Families who live far from the patient
- A discharge that feels thin on follow-up
Working alongside your NHS team
We do not replace your NHS GP, district nurse, consultant or A&E. We add private capacity at home. If you already have an NHS package, tell us so we do not duplicate or confuse the plan.
What we help with
- Understanding the discharge letter
- Medicines in the cupboard vs the list
- Wound and mobility checks
- Booking the right follow-up (GP, nurse, physio, NHS)
What happens during the visit
- Read the hospital letter with you
- Check the person, not just the paperwork
- Set a 7-day plan with named contacts
What is included
- Initial clinician visit
- Written 7-day plan
- Named phone contact during working hours
Who delivers your care
GP, ANP or nurse, matched to the discharge.
Pricing
From-price for an initial discharge visit and plan. Packages for the first 7 days are quoted after a short call.
After your visit
If the person is not safe at home, we say so. Private care does not replace 999 or a return to hospital.
Where we cover
Home visits across Lurgan, Craigavon, Portadown and Banbridge as core cover, with Lisburn, Armagh, Dungannon and Belfast arranged with a travel note. Call with a postcode and we will confirm. A family member can book if the patient consents to be seen.
FAQs
Can you collect someone from hospital?
Transport is arranged case by case. The clinical visit is the core of this service. Ask when you call.
