This is not an emergency service. If someone has chest pain, stroke signs, severe breathing difficulty, heavy bleeding or collapse, call 999 or go to A&E. We work alongside the NHS, not instead of it.
About this service
Well-managed diabetes is quiet. Poorly managed diabetes is not. Our home programme is regular glucose monitoring, medication and injection support, foot and skin vigilance, and the practical education that turns good control into habit. We work with your NHS diabetes team, not instead of them.
Who it is for
People with diabetes who struggle to attend clinic, older patients on insulin, and families supporting a relative whose control has slipped.

Working alongside your NHS team
We do not replace your NHS GP, district nurse, consultant or A&E. We add private capacity at home. If you already have an NHS package, tell us so we do not duplicate or confuse the plan.

Who delivers your care
A registered nurse or GP, depending on the visit. We will not start or stop insulin without a clear clinical plan.
Pricing
From-price for a clinician home visit in the diabetes programme. Ongoing packages are quoted after the first visit.
After your visit
Keep your NHS diabetes appointments. We can share readings with your GP or specialist with consent.
Where we cover
Home visits across Lurgan, Craigavon, Portadown and Banbridge as core cover, with Lisburn, Armagh, Dungannon and Belfast arranged with a travel note. Call with a postcode and we will confirm. A family member can book if the patient consents to be seen.
FAQs
Do you replace the NHS diabetes clinic?
No. We add capacity at home between those appointments. Retinal screening, structured education programmes and specialist insulin starts stay with NHS or agreed specialist teams.
